The Treatment of Selective Mutism in the Context of Evidence-based Practice: A Case of Clinical Application

|
This study is a case study of play therapy for a second-grader with selective mutism. The treatment consisted of 51 sessions and included cognitive-behavioral therapy techniques, sandplay therapy, and medication. Cognitive-behavioral therapy techniques, used in this case, included psychoeducation, exposure, shaping, contingency management, affective expression and modulation, and cognitive restructuring. Integrative psychotherapy and combined treatment of psychotherapy and medication were applied to this client after reviewing the literature regarding effective treatments for selective mutism and after taking account of this child¡¯s developmental status, characteristics, and the change process in therapy. This decision was made in the context of evidence-based practice(EBP) which is the synthesis of empirical evidence, clinical expertise, and patient values in implementing treatments. The child¡¯s symptoms were greatly improved by the end of the treatment process, and this assessment was based on the child¡¯s behaviors in therapy, self-reports, and reports from parents and teachers. Specifically, the child was able to communicate with others in social situations, became more spontaneous and playful, and accepted unexpected changes more easily.
